The Health Resources Services Administration's (HRSA) Bureau of Primary Health Care (BPHC) houses the Healthy Communities Access Program (HCAP) which provides grant funding to consortia of public and private healthcare providers-as well as social services, local government and other community-based organizations-to work together to develop or strengthen integrated community health care delivery systems that coordinate health care services for individuals who are uninsured or underinsured and to develop or strengthen activities related to providing coordinated care for uninsured or underinsured individuals with chronic conditions. Funding provided through HCAP builds upon and connects existing community resources and funding streams, bridging or eliminating the organizational, financial, and service delivery gaps that create fragmented systems of care for these populations. By supporting information sharing, as well as clinical, administrative and financial coordination among providers, the program builds systems of comprehensive and coordinated healthcare services. As one of the many coordination and integration strategies, some HCAP grantees include a focus on transportation as it represents a barrier for access to care. Some examples of HCAP activities involving transportation include: coordination of volunteer or negotiated transportation services for uninsured patients to get to appointments, providing patients with transportation vouchers or tokens and using information systems to arrange rides or refer clients to existing transportation programs which they are eligible to use; Grantees are permitted but not required by statute to use funds (where appropriate for the needs of the community) for "improvement to coordination of transportation to health care facilities". The specific dollar amount spent by HCAP grantees on transportation activities is not available, however, HCAP collectively receives approximately $83 million annually.
